Referrals can be made to all of the projects by; G.P,
Social worker, Key worker, Occupational Therapists (O.T's),
Community Mental Health Teams (CMHT's), Social Services
and other specialist agencies.
If you would like to make a
referral please contact the relevant project, you will
find project manager names and telephone numbers towards
the bottom of each page. The manager will send you a
Referral pack which needs to be completed by the referral
agent and the participant. Upon receiving the completed
pack the project manager will ask the referring agent
and the participant to attend a short, informal interview,
if everybody is happy, the participant will be given
a start date.
Please note that documentation
will be required as evidence that the participant is
eligible for funding - CPA 13 and has an agreed Care
Plan (CPA 9) prior to starting at any of our projects.
There are certain criteria
which must be met and in place prior to a referral being
made, these are:
